Medical Report

Medical reports are an essential and critical aspect of patient care. It encompasses the meticulous recording and analysis of patient information, medical assessments, treatment plans, and outcomes. These reports play a pivotal role in facilitating communication among healthcare professionals, tracking patient progress, and ensuring the delivery of effective and personalized medical care.

Patients, their families or authorized agents request Medical Report(s) for various purposes, such as for insurance claims and rebates, disability assessment, SOCSO Board, for second opinion or just for their retention. We will guide through the entire process of obtaining Medical Report(s) for our patients or their relatives/insurance agent.

How long does it take for my Medical Report to be ready?

It will take fourteen (14) working days for completion of medical report from the date receipt of complete set of documents and payment.

What are the documents required when I apply?
  • A complete set of application documents i.e insurance claim form/letter, SOCSO Form, EPF Form, etc.
  • Duly completed consent form for release of medical information from patient.
  • Attending Dr’s name.
  • Letter of authorization from patient if application is via third party.
  • Patient identification copy (IC)

How do I collect my Medical Report

You will be notified via phone or email. Kindly provide your contact details to our staff.

Can I ask someone to collect the report on my behalf?

Yes, but please provide the person with an authorization letter.

Can Thomson Hospital Kota Damansara post my report to my insurance company / SOCSO / EPF?

No. We will only release the report to the requestor due to reasons of confidentiality and to avoid dispute on proof of delivery.

Attending Physician Statement (APS) / Questionnaire

What is APS / Questionnaire?

APS is form or letter issued by the insurance company to obtain historical medical information of a patient who has an intention to purchase or renew an insurance policy. Questionnaire is a set of questions raised by the insurance company to obtain information an a patient's disease or condition such as hypertension, diabetes, gastritis, etc.

Do I need to give my consent for the report to be released to the insurer?

Yes, you need to give written consent for the released of medical information.

Can the insurance agent request and collect the report on my behalf?

Yes, you must authorize the agent in writing to collect on your behalf.

Insurance Claim / Reimbursement

Where do I get this form?

The form can be obtained from your insurance company or agent.

When do I submit the form?

After completion of your visit to the Outpatient Clinic or after discharge from the ward.

Do I need to sign the form?

Yes, it should be duly signed by the claimant or insured.

EPF Report

How do I apply for this report

You need to obtain the necessary forms(s) from EPF office or download it from the EPF website and submit it to us along with your request.

Can other family members (wife, children and siblings) request the EPF report on my behalf?

Yes, but you need to authorize them in writing to make the request.

SOCSO Report

You are advised to liaise with the SOCSO office pertaining to the type of withdrawal and fill in the correct and necessary form(s) for the application.
