What is Breast And Endocrine Surgery?

This surgical specialty encompasses two distinct yet related areas:

Medical Breast Surgery: The surgeon removes breast tumours and lymph nodes while preserving as much of the breast tissue as possible, focusing on the diagnosis and treatment of both benign and malignant breast conditions.

Endocrine Surgery: The surgeon specialises in the surgical treatment of disorders related to the endocrine system, a network of glands that produce hormones, including the thyroid, parathyroid, and adrenal glands.

Medical Breast Surgery

Common procedures include:

  • Mastectomy: Removal of all or part of the breast tissue
  • Lumpectomy: Removal of a breast lump and surrounding tissue
  • Biopsy: Removal of a tissue sample for examination
  • Sentinel lymph node biopsy: Examination of the first lymph nodes to which cancer cells are likely to spread
  • Axillary lymph node dissection: Removal of lymph nodes under the arm

The choice of surgery depends on the individual's condition and desired outcome, always prioritizing cosmetically acceptable results.

Endocrine Surgery

Specialises in surgeries of the:

  • Thyroid gland: Thyroidectomy (removal of all or part of the thyroid) for conditions like cancer, hyperthyroidism, and goitre
  • Parathyroid glands: Parathyroidectomy (removal of one or more parathyroid glands) for hyperparathyroidism (excessive parathyroid hormone production)
  • Adrenal glands: Adrenalectomy (removal of one or both adrenal glands) for adrenal tumours, pheochromocytoma, Conn’s and Cushing's syndrome.

Procedures can be performed using traditional open surgery or minimally invasive techniques (laparoscopy, robotics).

Multidisciplinary Breast Cancer Management

Breast cancer treatment involves a team of specialists: surgeons, medical oncologists, radiation oncologists, pathologists, radiologists, and breast care nurses. This collaborative approach ensures the most effective and personalized care.

Minimally Invasive Surgery (MIS)

MIS utilises small incisions, leading to:

  • Less pain and scarring
  • Reduced blood loss
  • Shorter hospital stays
  • Improved cosmetic outcomes

Techniques include:

  • Laparoscopy: Using a laparoscope (a thin tube with a camera) for visualization
  • Robotic surgery: Employing a robotic system for precise surgical movements
  • Endoscopic surgery: Utilising a flexible tube with a camera for internal examination
TOETVA – Scarless Thyroid Surgery

TOETVA (Transoral Endoscopic Thyroidectomy Vestibular Approach) is a minimally invasive thyroid surgery performed through small incisions inside the mouth, leaving no visible neck scars. It also offers potential benefits like reduced pain and faster recovery.
